SUMMARY:Bridging the Gap: Understanding the Needs of Ovarian Cancer Survivors
DESCRIPTION:There are more than 250\,000 ovarian cancer survivors in the U.S. and this number is growing. Throughout treatment for this disease\, many patients encounter serious effects\, including physical issues like nausea\, vomiting\, fatigue\, sleep disruptions\, peripheral neuropathy\, hair loss\, and sexual dysfunction\, as well as psychological challenges such as cognitive dysfunction\, fear of recurrence\, anxiety\, and depression. Comprehensively understanding how the prevalence and severity of side effects\, unmet needs\, and barriers to supportive care affect quality of life in diverse ovarian cancer survivors remains a research gap and priority.

SUMMARY:Standing Up for LGBTQIA+ Voices in Aging and Alzheimer’s Disease Research
DESCRIPTION:Over 3 million adults aged 60+ in the U.S. identify as lesbian\, gay\, bisexual\, transgender\, queer\, intersex\, or another LGBTQIA+ identity. Despite this\, there are numerous health and research needs that would most benefit the LGBTQIA+ aging community and those who support them. In this talk\, we will examine the unique risks and challenges LGBTQIA+ older adults face related to aging and Alzheimer’s disease\, addressing current research gaps and challenges within today’s sociopolitical climate. Drawing on lessons learned from our national research studies with LGBTQIA+ older adults\, we will discuss key insights and future directions. We will also highlight the importance of collecting sexual orientation and gender identity (SOGI) data and why the historical and current political climate requires us to stand up for LGBTQIA+ aging communities and advocate for inclusive public health research.

SUMMARY:Disparities in Pediatric Leukemia: From Pre-Diagnosis to Survival
DESCRIPTION:Disparities exist along the pediatric leukemia continuum\, from incidence to outcome. Of note\, despite improvements in childhood cancer survival in the last several decades\, marked racial\, ethnic\, and socioeconomic disparities in outcomes persist. Compared with non-Hispanic white children\, non-Hispanic black and Hispanic children experience lower survival from many cancers\, including leukemia\, the most commonly diagnosed cancer in children. The underlying causes of these survival differences are poorly understood and may vary by cancer type\, and both biological and socioeconomic pathways have been proposed. Recent evidence has suggested that lower socioeconomic status (SES) is associated with survival from some childhood cancers. This seminar will explore disparities along the cancer continuum\, discuss potential underlying mechanisms\, and share ongoing work characterizing the impact of socioeconomic status on outcome.

DESCRIPTION:Leveraging Data Analytics and Disease Modeling to Support Public Health Decision Making for Parvovirus B19 Outbreaks \nThe Midwest Analytics and Disease Modeling Center (MADMC) was founded in order to build on the lessons of COVID-19 to be better prepared for future public health emergencies. It is a partnership between the University of Minnesota\, the Minnesota Department of Health\, and the Minnesota Electronic Health Records Consortium (MNEHRC)\, which brings together the 11 largest health systems in Minnesota that collectively provide care to over 90% of Minnesotans. As part of the national InsightNet network funded by the Center for Forecasting and Outbreak Analytics at the US Centers for Disease Control and Prevention\, MADMC will develop modeling and analytic infrastructure so that the Midwest\, and the nation\, will be better able to obtain and analyze data to help decision-makers weigh their options in a future public health emergency.

DESCRIPTION:South Carolina Statewide Electronic Health Records and its Modeling Cases \nPresented by Jiajia Zhang\nDepartment of Epidemiology & Biostatistics\nUniversity of South Carolina \nElectronic health records (EHRs) are promising but challenging resources for research on investigating and monitoring disease progression. Due to their complexity\, we proposed several ways to address longitudinal or unstructured EHRs data to improve the dynamic prediction of survival outcomes. \nCase 1: Motivated by improving the prediction of the HIV suppression status using electronic health records (EHR) data\, we propose a functional multi-variable logistic regression model that accounts for the longitudinal binary process and continuous process simultaneously. Specifically\, the longitudinal measurements for either binary or continuous variables are modelled by functional principal components analysis and their corresponding functional principal component scores are used to build logistic regression model for prediction. The longitudinal binary data are linked to underlying Gaussian processes. The estimation is done using penalized spline for the longitudinal continuous and binary data. Group-lasso is used to select longitudinal processes and multivariate FPCA is proposed to revise functional principal component scores with correlation. The method is evaluated via comprehensive simulation studies and then apply to predict viral suppression using EHR data for people living with HIV in South Carolina. \nCase 2: Comorbidity indices like the Charlson Comorbidity Index (CCI) provide a single score but overlook the impact of comorbidity history/timing and severity features on mortality. We propose dynamically predicting the risk of all-cause mortality using a landmark large language model that could decipher the chronological comorbidity history as well as consider the severity. The data included 45\,353 hospitalized patients in Prisma Hospital in South Carolina. We propose a two-step landmark large language model: initially extracting longitudinal features from concatenated comorbidity descriptions history via BERT (Bidirectional Encoder Representations from Transformers)\, followed by fitting a binary classification model to predict all-cause mortality of patients. \nThe results provide evidence that EHRs and dynamic prediction have the potential to assist clinicians in understanding patients’ disease progression based on their historical information at different times. \nA seminar tea will be held at 2:45 p.m. in University Office Plaza\, Room 240. What and how we communicate as epidemiologists can influence replication\, accuracy of interpretation\, and translation of science into policy. Moreover\, our use of language reflects our thinking and value systems\, reifying structures in society through science. This talk will cover the state of practice for communicating race and ethnicity in multiple health disciplines\, how current communication practice threatens both scientific rigor and equity\, and the possible actions at the individual and structural level we can take to address these issues.
