SUMMARY:BHDS Seminar with Jiajia Zhang
DESCRIPTION:South Carolina Statewide Electronic Health Records and its Modeling Cases \nPresented by Jiajia Zhang\nDepartment of Epidemiology & Biostatistics\nUniversity of South Carolina \nElectronic health records (EHRs) are promising but challenging resources for research on investigating and monitoring disease progression. Due to their complexity\, we proposed several ways to address longitudinal or unstructured EHRs data to improve the dynamic prediction of survival outcomes. \nCase 1: Motivated by improving the prediction of the HIV suppression status using electronic health records (EHR) data\, we propose a functional multi-variable logistic regression model that accounts for the longitudinal binary process and continuous process simultaneously. Specifically\, the longitudinal measurements for either binary or continuous variables are modelled by functional principal components analysis and their corresponding functional principal component scores are used to build logistic regression model for prediction. The longitudinal binary data are linked to underlying Gaussian processes. The estimation is done using penalized spline for the longitudinal continuous and binary data. Group-lasso is used to select longitudinal processes and multivariate FPCA is proposed to revise functional principal component scores with correlation. The method is evaluated via comprehensive simulation studies and then apply to predict viral suppression using EHR data for people living with HIV in South Carolina. \nCase 2: Comorbidity indices like the Charlson Comorbidity Index (CCI) provide a single score but overlook the impact of comorbidity history/timing and severity features on mortality. We propose dynamically predicting the risk of all-cause mortality using a landmark large language model that could decipher the chronological comorbidity history as well as consider the severity. The data included 45\,353 hospitalized patients in Prisma Hospital in South Carolina. We propose a two-step landmark large language model: initially extracting longitudinal features from concatenated comorbidity descriptions history via BERT (Bidirectional Encoder Representations from Transformers)\, followed by fitting a binary classification model to predict all-cause mortality of patients. \nThe results provide evidence that EHRs and dynamic prediction have the potential to assist clinicians in understanding patients’ disease progression based on their historical information at different times. \nA seminar tea will be held at 2:45 p.m. in University Office Plaza\, Room 240. All are Welcome.

SUMMARY:CHAI Speaker Series: Aging & Disability
DESCRIPTION:Older and younger adults with disabilities share common challenges\, from navigating inaccessible environments to finding care providers who understand the intersection of disability\, health\, and wellness. As more people age with long-term or lifelong disabilities\, there is a growing need to bridge aging and disability research\, practice\, and policy. \nThis free webinar will begin with a presentation by national expert\, Dr. Michelle Putnam\, exploring the importance of bridging aging and disability\, why this work is critical now\, and key actions needed to advance it. A panel of local experts from Minnesota will then share their perspectives\, highlighting real-world challenges and opportunities for collaboration. \nView previous CHAI Speaker Series events

SUMMARY:Threats to Equity: Race and Ethnicity Communication Practices and Journal Standards across Health Disciplines
DESCRIPTION:Language is a powerful tool that can clarify or obscure; justify or delegitimize. What and how we communicate as epidemiologists can influence replication\, accuracy of interpretation\, and translation of science into policy. Moreover\, our use of language reflects our thinking and value systems\, reifying structures in society through science. This talk will cover the state of practice for communicating race and ethnicity in multiple health disciplines\, how current communication practice threatens both scientific rigor and equity\, and the possible actions at the individual and structural level we can take to address these issues.

SUMMARY:Three Ways of Looking at Black/White Mortality Differences in the United States
DESCRIPTION:A basic demographic fact about the United States is that\, on average\, White people live longer than Black people. But what\, concretely\, does this fact mean? Drawing from a diverse set of social science\, health\, and literary sources\, this talk argues that social science research has three families of strategy for making sense of the size of mortality disparities — distribution-based\, action-based\, and meaning-based measures — and provides new empirical results in each vein that collectively aim to put demographic measurement onto a more human footing.

SUMMARY:Firearm Injury in the United States: Facts\, Myths\, and a Public Health Path Forward
DESCRIPTION:Dr. Rebecca Cunningham is one of the nation’s leading experts on issues related to firearm injury prevention. Over the past 20 years\, Dr. Cunningham has secured more than $50 million in federal grants to advance evidence-based research projects designed to address the root causes of\, and potential solutions for\, the most important issues involving firearm violence. She served as director of the U.S. Centers for Disease Control and Prevention-sponsored Injury Prevention Center at the University of Michigan. Dr. Cunningham also launched the Institute for Firearm Injury Prevention at Michigan to support and catalyze innovative research and train future generations of scientists to pursue this field of scholarship. She also served as principal investigator on the Firearm Safety Among Children and Teens Consortium\, sponsored by the National Institutes of Health\, which brings together researchers\, practitioners and firearm owners to identify opportunities to reduce youth firearm injuries and deaths.

SUMMARY:BHDS Seminar with Tanya P. Garcia
DESCRIPTION:Robust and Efficient Estimation in the Presence of a Randomly Right-censored Covariate \nPresented by Tanya P. Garcia\nDepartment of Biostatistics\nUniversity of North Carolina at Chapel Hill \nIn Huntington’s disease research\, a current goal is to understand how symptoms change prior to a clinical diagnosis. Statistically\, this entails modeling symptom severity as a function of the covariate “time until diagnosis\,” which is often heavily right-censored in observational studies. Existing estimators that handle right-censored covariates have varying statistical efficiency and robustness to misspecified models for nuisance distributions (those of the censored covariate and censoring variable). On one extreme\, complete case estimation\, which utilizes uncensored data only\, is free of nuisance distribution models but discards informative censored observations. On the other extreme\, maximum likelihood estimation is maximally efficient but inconsistent when the covariate’s distribution is misspecified. We propose a new “SPARCC” estimator (for Semi-parametric Right-Censored Covariate) that is robust and efficient. When the nuisance distributions are modeled parametrically\, the SPARCC estimator is doubly robust\, i.e.\, consistent if at least one distribution is correctly specified\, and semiparametric efficient if both models are correctly specified. When the nuisance distributions are estimated consistently via nonparametric or machine learning methods\, the estimator is consistent and semiparametric efficient. We show empirically that the proposed estimator\, implemented in the R package sparcc\, has its claimed properties\, and we apply it to study Huntington’s disease symptom trajectories using data from the Enroll-HD study. \nA seminar tea will be held at 2:45 p.m. in University Office Plaza\, Room 240. All are Welcome.
